Maternity
This benefit Includes routine newborn care
and is paid at 60% after separate $1000 deductible for an
in-network provider. The medical out-of-pocket maximum does
not apply. A waiting period may apply. Benefit may vary by
state.
Supplemental Accident Benefit
You can choose either a $500 or a $1,000
benefit, and each person on the policy receives the benefit.
The benefit pays the first $500 or $1000 per accident at
100%, and then the base plan benefit applies. The treatment
of an accident must be administered within 90 days of the
accident.
Dental
HumanaOne makes it easy and
affordable to add dental benefits to your health plan
through the same application process. If you are approved
for a health plan they are approved for dental as well.
Benefits include:

Preventive services — 100%, no deductible, including
oral examinations and routine cleanings
•
Basic services — 50% after deductible
•
Teeth whitening services — 50% after deductible (not
available in Florida)
•
Major services — 50% after deductible (Waiting period
applies.
•
Orthodontia discount — up to 20% discount if they
visit an orthodontist from the Humana Dental PPO Network and
ask for the discount
Annual Deductible:
• $50 individual • $150 family
Annual maximum benefit • $1,000
►Dental is not available in Indiana.
About
the Rx4 Prescription Drug Benefit
Each covered prescription drug is assigned to one of four
levels with a different co-payment for each level.
•
Level One – Lower-cost generic and some brand-name
drugs
•
Level Two – Higher-cost generic and some brand-name
drugs
•
Level Three – Higher-cost, mostly brand-name
alternatives to drugs on Level One and Two.
Also includes some self-administered
injectable medications.
•
Level Four – Highest cost, includes self-administered
injectable medications and high technology drugs
For levels two through four, once the drug
deductible is met, you are only responsible for a co-payment
and the plan pays the remaining cost for drugs filled at a
participating pharmacy.
For level one drugs, you are only
responsible for the co-payment.

product filing that the rate not be changed by any sales
entity; therefore, rates will be the same from all
websites. This assures the buyer of always
receiving the best rate on any and all insurance
products.
