Quality Point-of-Service^® (QPOS^®), USAccess^® and Aetna Choice^® POS plans are HMO wrap products. In-network/referred benefits are underwritten or administered by the HMO: Aetna Health Inc., Aetna Health of California Inc., Aetna Health of the Carolinas Inc. or Aetna Health of Illinois Inc. Out-of-network benefits are underwritten or administered by Aetna Health of California Inc., Corporate Health Insurance Company or Aetna Health Insurance Company of New York. In some states the HMO also underwrites or administers the out-of-network/nonreferred benefits through Aetna Health Inc., or Aetna Health of California Inc. Self-funded plans are administered by Aetna Life Insurance Company and Aetna Health Administrators, LLC.

Out-of-network/nonreferred indemnity benefits are underwritten by Corporate Health Insurance Company. Aetna HealthFund^®, Elect Choice^®, Open Access Elect Choice, Managed Choice^® POS, Open Access Managed Choice^®, Open Choice^® PPO, Affordable Health Choices from Aetna^SM Supplemental Insurance Plan and Traditional Choice^® medical plans are underwritten or administered by Aetna Life Insurance Company. Elect Choice plans are only available on a self-funded basis, except in NY and WA.

Group Life Insurance, Group Long Term Care Insurance and Group Disability Insurance are underwritten by Aetna Life Insurance Company.

In Arizona Aetna Advantage^™ Dental, Basic Dental and Family Preventive Dental Plans are provided or administered by Aetna Health Inc. PPO Dental plans and Indemnity Dental plans are underwritten or administered by Aetna Life Insurance Company.

Dental Maintenance Organization^® (DMO^®)*, Aetna Advantage^™ Dental and Basic Dental are underwritten or administered by Aetna Life Insurance Company, Aetna Dental Inc. * State laws vary with regard to out of network benefits. In Illinois, DMO plans provide limited out-of-network benefits. However in order to receive maximum benefits, members must select and have care coordinated by a participating primary care dentist. Illinois DMO is not an HMO.
