Oral health affects both physical and mental wellness – and therefore quality of life. That’s why Anthem Blue Cross and Blue Shield offers the Anthem Blue Dental PPO Plan for individuals and families. 

We designed our PPO dental plan with two goals in mind: to promote good dental hygiene and preventive care, important elements in a total health care package, and to provide you with the dental benefits you need in a convenient, cost-conscious manner. 


How the Plan Works

When you choose and in-network dental provider, you’ll receive care at Anthem Blue Cross and Blue Shield’s negotiated discounted rates. If you choose and out-of-network provider, we still provide benefits, but your out-of-pocket expenses may be higher, because our negotiated fees don’t apply to out-of-network providers.

Broad, Convenient Provider Network

The PPO Plan features access to nearly 60,000 credentialed dental providers nationwide, including general practitioners and specialists. The PPO network includes more than 400 providers in Nevada.   


Benefits: Dental PPO Plan for Individuals and Families

The plan features preventive and diagnostic care, basic dental care, and major dental care. Coverage includes: Routine Check-ups, Cleanings, X-Rays, Filling, Crowns, Dental Surgery, Gum Disease Treatment, Root Canals, Crowns and Dentures

Anthem Blue Plans at a Glance


Anthem Blue 5000
– Best for healthy families who expect few or no health care expenses. The plan includes coverage for office visits, preventive care for adults, babies and children, lab work and X-rays, and more. This plan has a higher deductible with a low monthly premium. 


Anthem Blue Saver 2000
– For individuals and families interested primarily in hospital coverage and reduced outpatient benefits. It has a lower deductible and monthly premium. 


Anthem Blue HSA 2600
– For individuals and families who expect few or no health care expenses. The plan is a high-deductible health plan that meets federal guidelines for clients who want to establish a health savings account to save for future qualified medical expenses. 


Network or Non-network: The Choice is Yours.
Whether you choose from our extensive preferred provider network, or choose to visit a non-network provider, Anthem Blue plans help keep your costs manageable. Anthem Blue plans do offer coverage for most care received from non-network providers. However, to receive the highest level of benefits, choose a provider who participates in the Anthem Blue provider network


Comprehensive, Referral-free Care.
Anthem Blue plans offer referral-free care from a preferred provider network of more than 3,200 health care providers and 21 hospitals in Nevada. You may also visit providers outside the network. For these visits, you will pay a greater share of the cost.
