 ■  Are the plan benefits comprehensive?
Some policies only cover doctor charges. Others may only cover hospital charges.

■  Does the plan include the doctors that you are currently seeing?
In other words, is your doctor or preferred hospital in the network. A consideration for
PPO and HMO plans. 

■  Are the health care benefits that you need covered? 
For example, maternity. Individual policies may automatically include maternity benefits
for females within a certain age even if they do not want or need the coverage. Other
plans offer maternity only as an added benefit and increased premium rate.

 ■  Does the plan cover pre-existing conditions?
Preexisting condition exclusions are generally limited to a 12-month maximum after enrollment
for conditions that were treated or diagnosed in the previous six months.

■  How much are the co-pays and deductibles?
This will influence the price of the policy.

 ■  Does the plan include dental care, vision care, preventative care, maternity care, prescription medication, and mental health care?
